The Centers for Medicare & Medicaid Services has released its October 2025 refresh of skilled nursing facility quality data on Medicare’s Care Compare website, marking the first public reporting of three new assessment-based measures. The updated data provides consumers and families with expanded information for comparing facilities when making care decisions.

New Measures Debut

The October refresh introduces public reporting of Transfer of Health Information to Provider (TOH-Provider) and Transfer of Health Information to Patient (TOH-Patient) measures, which assess whether facilities share critical health information during care transitions. National performance on TOH-Provider averages 84%, while TOH-Patient averages 78%, indicating room for improvement in patient-directed information sharing.

The refresh also debuts the COVID-19 Vaccination Status measure, reporting the percentage of residents who are up-to-date on COVID-19 vaccinations. National performance varies significantly by region, ranging from 62% in some Southern states to over 85% in the Northeast.

Star Rating Impacts

While the new measures are currently reported for informational purposes only, CMS has indicated they may be incorporated into the Five-Star Quality Rating System in future years. Facilities should monitor their performance on these measures and implement improvement strategies proactively.

The October refresh also updates existing quality measures with Q2 2025 data, including long-stay measures for falls, pressure ulcers, and antipsychotic medication use, and short-stay measures for functional improvement and discharge to community.

Updated quality data is available on Medicare’s Care Compare website and through the Provider Data Catalog for researchers and analysts. CMS encourages consumers to use quality data alongside inspection reports and staffing information when evaluating facilities.
